Required Documents Checklist
- Signed Application (every line filled in)
- Birth Certificate (including siblings)
- Immunization Record
- Parents' Driver's License
- Proof of Residence - utility bill, lease, or letter of temporary residence
- Proof of Income for each adult in household - pay stubs (4), college transcript, hire account, or statement of no income
- Unearned Income (if applicable) - social security, alimony, etc.
- Primary Language Survey
